Every patient in our program is assigned a case manager, an individual who will be that person's
guide throughout the course of their treatment. The case manager's role is to assist the patient through the process of recovery, coordinating the various system components in order to achieve a
successful outcome.
The five case management activities are:
1. assessment
2. planning
3. linking
4. monitoring
5. advocacy
Case management is a service that helps people to organize appropriate services and supports.
A case manager coordinates mental health, social work, educational, health, vocational, transportation, advocacy, respite care, and recreational services, as needed.
